About the Program

Community Partners is a home-based case management program.  Families can receive up to four months of support and services in this program.  Services are tailored to the family's unique needs and goals.

Hours

Monday - Friday 8:00 am - 5:00 pm

Who is Community Partners for? 

​Community Partners is designed for families with a child newborn up to 18 years old and not currently enrolled in Healthy Families.

What can families expect?

This program helps families become self-sufficient by utilizing the resources available in the community and ensuring that families have the support they need to be successful. These resources could include, but are not limited to hospitals, faith-based organizations, schools, and First Steps. The staff will identify the resources, make the referrals, and assist families in accomplishing their goals by working together.
