Seeking more information about your adoption?


Who can request and get non-identifying information:

  • Adopted adults
  • Birth parents 
  • Adoptive parents

How to request information:

Submit a letter to:

Request letter should include:

  • Your name (if adopted child include name of adopted parents)
  • Date of birth
  • What type of information you are requesting (non-identifying information, medical or both)
  • A signature and an ID with your signature such as driver's license
Once we receive the letter and retrieve your file the information you requested will be compiled and sent to you. We also include the State of Indiana Adoption Registry Forms for you to complete if you wish to be registered. The forms need only be completed if you wish to connect with your birth parents/child.
